Safeguarding adult referrals

The safeguarding adults electronic referral form launched in July 2024 is for referrals from professionals only.

Members of the public who would like to raise a safeguarding concern should continue to contact Call Derbyshire telephone: 01629 533190.

The electronic referral form can be saved at the point of submission by the referrer to allow referring agencies to store a copy of the safeguarding adult referral on their own recording systems.

The Word document option for submitting referrals is still available but we strongly encourage referrers to use the new electronic version of the form as it is designed to be much more user friendly and will also assist with improving data capture and data quality.

Important: Electronic referrals forms will only be reviewed and processed Monday to Friday (excluding Bank Holidays) from 9am to 5pm. If your safeguarding referral requires a response out of hours, please contact Call Derbyshire telephone: 01629 533190. If you gain more information or updates that may change your referral's priority, or you feel that your referral requires a same day response, please contact Call Derbyshire telephone: 01629 533190.

Is this the right referral route for you?

A safeguarding adult referral is the right route where you are concerned that the person is experiencing or at risk of abuse or neglect. If you are a professional requesting an adult care assessment for someone you work with, please use the online professional referral form.

If you are asking for help for yourself, a friend or family member, you can complete an online self-assessment, or ask for an assessment from adult social care where it appears that you may have need for care and support.
